The short answer

Vixen's Colossus ($144.25) is the best penis sleeve for most men, with platinum silicone, a solid head, a ball strap and an inside 5⅜ inches long. Of the three I've worn, Pipedream's Mega 2" fit best and the Oxballs Butch stayed put best.

The Short Version

A penis sleeve goes over an erection and adds length, girth or both. The best one for most men is Vixen's Colossus, $144.25 at SheVibe. It's platinum silicone, ribbed inside, with a solid head and a strap that goes behind the balls. Inside, it's 5⅜ inches long and made to stretch in width. I'm 5.3 inches, which is about as close a match as a sleeve gets.

I've worn three myself. Pipedream's Mega 2" Extension fit best and felt best, the Oxballs Butch stayed put best, and XR's XL Black Mamba was fine. None of the three is silicone, which is why the list starts with the ones that are.

Prices read on October 31, 2026 at the store each link goes to, before tax and shipping. SheVibe for the Vixen and Pipedream Elite sleeves (Vixen's own prices are $156 for the Colossus and $148 for the Ride On, and Pipedream's are $82.99 and $149.99), Amazon for the Lovehoney, sold by Lovehoney LLC, and Adam & Eve for the Mega 2" ($54.99 at Pipedream) and the Butch ($72 at Oxballs). Adam & Eve usually runs a code for half off one item. Inside sizes are the makers' own, except the Lovehoney canal, which comes from its Amazon listing.

Strokers are a different toy, and the pocket pussy guide covers those. For prostate toys, rings and the rest, see our ranking of the best male sex toys.

Sleeve, Sheath or Extension?

All three words mean a hollow shaft you wear over your penis, and stores use them almost interchangeably. Where there is a difference, it's this. An extension has a solid section past your tip, so it adds length as well as girth. A girth sleeve adds thickness and sometimes leaves the head open. Oxballs, Bad Dragon and XR call theirs sheaths. Most come with a strap or ring that goes behind the balls.

One word trips people up. The soft insert in a Fleshlight is also called a sleeve, and so is any stroker you slide over yourself. Those are masturbators. This page is about the ones you wear during sex.

You'll find four kinds.

Extensions with a solid tip add one to three inches. Pipedream's Elite line puts the extra length in a removable extender, so you can take it out and wear the sleeve for girth alone.

Girth sleeves add thickness, and an open tip keeps the head of your penis free. Bad Dragon's Crackers' Sheath ($70 to $90 in three sizes) is open at the end, in Bad Dragon's soft silicone.

Firm sleeves work without an erection. Vixen's Ride On is the one I'd look at.

Vibrating sleeves put a motor in the tip or along the shaft. Pipedream's vibrating Elite has 10 modes and a remote.

A hollow strap-on is the same idea with a harness carrying it, and our strap-on ranking covers those.

How to Get the Fit Right

Fit decides whether a sleeve works, and the good makers publish the numbers you need. Measure your erection along the top, from the pubic bone to the tip, and around the middle. Then compare those with the sleeve's inside, not its outside.

Length

Vixen's advice is the clearest I've read. The head of your penis should sit right behind the sleeve's solid tip. A gap of more than an inch inside makes the sleeve buckle when you thrust, and if you're more than an inch longer than the inside, it gets pushed forward. Vixen sells an insert you cut to fill a gap, and the Butch comes with a bullet-shaped insert for the same job.

Width

You want the inside a little narrower than you, so the sleeve grips as it stretches. The Colossus is 1⅜ to 1½ inches across inside and made to stretch in width. Pipedream says its sleeves stretch to fit most sizes. Oxballs prints a usable range for each sheath, 5 to 6 inches around for the Butch. The Ride On barely stretches, and Vixen sends larger men to the Colossus or its Holster instead.

What holds it on

A strap or loop behind the testicles keeps the sleeve from sliding off on the outstroke. Without one, it relies on grip and lube. Pipedream says to trim the base of the Mega with scissors for a better fit, and never while you're wearing it.

Open or closed tip

A solid tip adds length and covers the head, where most of the feeling is. An open tip keeps it free and adds girth only.

The Three I've Worn

Pipedream Mega 2" Extension

The best fit and the best feel of the three. It's TPE (thermoplastic elastomer)A soft, cheap material used for most stroker sleeves. It's porous, so it can't be fully sanitized, and a sleeve lasts about a year of regular use. More in the glossary, which Pipedream calls Fanta Flesh, and in the hand it feels a bit cheap. On, it's a different toy. It adds 2 inches at the tip, and Pipedream says it makes you 66% thicker. I lose some feeling in it, as in any sleeve with a closed tip, and I still came inside it the last time I wore it with Sasha. That says more about the fit than a spec sheet does. Adam & Eve sells it for $64.99, and Pipedream's own store for $54.99.

Oxballs Butch

I liked this one better than the Mamba. It's thick, veiny TPR with a sling at the base, one ring for the shaft and one for the balls, and Oxballs lists both at 3.5 inches around. Getting through both took some work. Once it was on, it didn't move. Oxballs prints a usable range of 5 to 6 inches around for the Butch, and I'm 4.3, so on me the sling did the holding. Oxballs sells it for $72, and Adam & Eve for $84.99.

XR XL Black Mamba

It was fine. It's black PVC with nubs inside and a ring for the balls, $23.95 at XR's own store. I liked the Butch better, and neither of XR's pages for the Mamba says it's PhthalatesSofteners used in PVC and jelly toys, linked to hormone disruption. Silicone, glass, stainless steel and ABS plastic don't contain them. More in the glossary-free, so it isn't one of the picks.

Materials and Safety

Most sleeves are TPE, TPR or PVC, because those stretch, and the better ones are silicone. Planned Parenthood lists TPE, TPR, PVC, jelly and silicone blends among the materials that may be Porous and nonporousPorous materials like TPE, jelly and Cyberskin have tiny pores that can hold bacteria. Nonporous ones like silicone, glass and steel can be cleaned completely. More in the glossary and can hold germs even after you wash them. In a small 2014 study, HPV was still on a TPE vibrator in two of five tests a day after cleaning, and gone from the silicone one.

So a TPE or TPR sleeve stays with one person, and silicone is what to buy if you want one that washes clean. Pipedream says its Elite silicone can go in boiling water for three minutes, though not the vibrating extender. Lovehoney sells silicone and plastic sleeves side by side under the same name, so read the material line. The Mega Mighty on this list is silicone, while its Mega Thick ($36.99) and the 1 to 3 inch Mega Mighty ($42.99) are listed as plastic on Amazon.

PVC needs plasticizers to be soft, and some of those are phthalates. California lists DEHP, the best known of them, as causing cancer and male reproductive harm. Oxballs says its flexTPR is phthalate free. XR says nothing either way about the Mamba. The body-safe materials guide has the long version.

A sleeve isn't a condom. Pipedream says its sleeves don't protect against pregnancy, HIV or other STIs, so ignore any listing that says otherwise. If a sleeve goes from one partner to another, put a fresh condom on it for each, as Planned Parenthood advises for any shared toy.

Pipedream also warns that long and frequent use of penis-enhancing devices can rupture blood vessels and cause bleeding and bruising under the skin, and can make Peyronie's disease, priapism and urethral stricture worse. If you have any of those, ask a doctor first.

How to Put One On

Wash it first.

Warm water and a mild antibacterial toy cleaner, as Pipedream says, or soap and water for silicone.

Trim it only if the maker says you can.

Pipedream's sleeves trim at the base. Do it with scissors, before you put it on.

Follow the maker on putting it on.

Pipedream folds its Elite sleeves inside out so the extender goes into the tip first, then rolls the sleeve down your shaft. Vixen says never to roll its sleeves down or turn them inside out. They're built differently, so do what the box says.

Lube the inside and yourself.

Water-based lube is the one every maker here allows. Pipedream and Vixen say no silicone lube on their sleeves, and Oxballs allows it on flexTPR. The silicone lube guide explains why makers split on it.

Put it on half-hard.

Medical News Today suggests putting a sleeve on before you're fully hard, so it's snug at full erection. Squeeze the air out toward the base as you go, as Oxballs suggests, and get your head right behind the tip.

Balls through the loop.

Stretch the strap and bring it behind the testicles. It should hold, not hurt.

Lube the outside too.

Your partner gets the outside, so treat it like any toy and use plenty.

Treat the strap like a cock ring.

Most makers say a constriction ring shouldn't stay on longer than 30 minutes, according to the ISSM. Take it off at the first numbness, pain or coldness. Our cock ring ranking has the rest of the safety rules.

Expect to feel less.

A closed tip covers the head, and most of the feeling goes with it. Some men wear a sleeve to last longer for exactly that reason, and an open tip is the answer if you'd rather feel more.

Sleeves and Erectile Dysfunction

A soft sleeve needs an erection to work. Vixen says so about the Colossus. If erections don't hold, you want a firm sleeve, and the Ride On is the one I'd look at. Vixen made it for men with ED, a smaller penis, or to keep going between erections. It's firm enough to do the work for you, it barely stretches, and it's $137 at SheVibe.

A hollow strap-on does the same job with a harness carrying the weight. Doc Johnson's Body Extensions are our pick there, with hollow silicone shafts 1.6 inches wide inside before they stretch.

The evidence for all of this is thin. A 2017 review in Translational Andrology and Urology found that sleeves can work with ED depending on their size and stiffness, and that most of the evidence is anecdotal. If erections have changed, see a doctor before you buy anything. For solo play, the Hot Octopuss Pulse works soft, half-hard or fully erect.

Cleaning and Storage

Wash it right after you take it off. Silicone takes warm water and mild soap, and Pipedream's Elite silicone can be boiled for three minutes. For TPE and TPR, follow the maker. Pipedream says warm water and an antibacterial toy cleaner on the Mega, with no boiling and nothing harsh or alcohol-based. Oxballs says warm soapy water for flexTPR.

Dry it completely before it goes away, inside included. Pipedream says never to store one damp, and a damp TPE sleeve is how mold starts.

Store it on its own. Pipedream, Oxballs and Doc Johnson all say to keep these apart from other toys, and TPE and PVC can leave silicone tacky where they touch. Oxballs suggests a cloth pouch or a cotton sock.

Replace a TPE or TPR sleeve when it stays tacky after washing, keeps a smell, tears or shows spots inside. The cleaning guide covers every material.

What to Skip

PVC sleeves that don't say what's in them. If a listing names no material, or says nothing about phthalates, buy one that does.

Anything sold as a condom. A sleeve isn't one.

Unbranded Amazon sleeves. A listing with no maker website, no material named and a smell out of the box is a gamble with sensitive skin. Lovehoney's own listings, sold by Lovehoney LLC, at least come from a maker you can look up.

A sleeve much longer than you. More than an inch of empty space inside and it buckles. Fill the gap with an insert or buy a shorter one.

Silicone lube on a silicone sleeve, unless the maker says yes.

A strap left on too long. Thirty minutes at most, and off at the first numbness.

Sharing a TPE sleeve. It can't be sterilized, so it stays with one person.

What I'd Buy

If you buy one sleeve, make it the Colossus, and measure yourself before you order, because with sleeves the fit is everything. If $144 is a lot for a first try, Lovehoney's silicone Mega Mighty costs $26.99 and tells you whether you like the idea. For erections that don't hold, the Ride On. For the most length plus a motor, the vibrating Elite.

In my own drawer, the Pipedream is the one that fits and the Butch is the one that stays where I put it.

WINNER: Vixen Colossus
The best penis sleeve for most men: $144.25, platinum silicone, a solid head and a ball strap, with an inside 5⅜ inches long. Start with Lovehoney's $26.99 Mega Mighty if you want to try the idea first, and pick Vixen's firm Ride On if erections don't hold.

Frequently Asked Questions

What is the best penis sleeve?

Vixen's Colossus ($144.25 at SheVibe), for most men. It's platinum silicone with a solid head, a ball strap and an inside 5⅜ inches long that stretches in width. Lovehoney's Mega Mighty ($26.99) is the cheapest silicone one, Vixen's Ride On ($137) is the firm one for erectile dysfunction, and Pipedream's vibrating Elite ($114) adds a motor.

What is a penis sleeve?

A hollow shaft you wear over an erect penis to add length, girth or both. Stores also call them extensions, extenders, sheaths and girth enhancers. Most have a strap or ring behind the balls to hold them on. A Fleshlight sleeve is something else, a stroker for masturbation.

Do penis sleeves work?

They add the length and girth the maker states, as long as they fit. The inside should be about your length and a little narrower than you. You feel less through a closed tip. For erectile dysfunction, only a firm sleeve like Vixen's Ride On works without an erection, and the evidence for sleeves is mostly anecdotal.

Can you cum in a penis sleeve?

Yes. I came inside Pipedream's Mega 2" the last time I wore it. Wash it right after and let it dry completely. A sleeve isn't a condom, though, and Pipedream says its sleeves don't protect against pregnancy or STIs.

Are penis sleeves safe?

A branded one is, used with water-based lube, with the strap off within 30 minutes and cleaned after every use. TPE and TPR sleeves are porous, so keep each to one person. Skip PVC sleeves that say nothing about phthalates. Pipedream warns that long and frequent use can rupture blood vessels and make Peyronie's disease worse, so ask a doctor first if that applies to you.

What's the difference between a penis sleeve and an extension?

An extension has a solid section past your tip, so it adds length as well as girth. A girth sleeve adds thickness and may be open at the tip. A sheath is the word Oxballs, Bad Dragon and XR use for the same idea. Stores mix all three words up, so read what the toy adds.

How do I choose the right size penis sleeve?

Measure your erection along the top and around the middle, then compare with the sleeve's inside. Vixen says your head should sit right behind the solid tip, a gap of more than an inch makes a sleeve buckle, and being more than an inch longer than the inside pushes it forward. The inside should be a little narrower than you, and some makers print a usable range.

Do penis sleeves help with erectile dysfunction?

Firm ones can. Vixen's Ride On ($137) is made to work without an erection, while soft sleeves like the Colossus need one. A hollow strap-on does the same job with a harness. A 2017 review found the evidence mostly anecdotal, so see a doctor if erections have changed.

What lube should I use with a penis sleeve?

Water-based, which every maker on this page allows. Pipedream and Vixen say no silicone lube on their sleeves, while Oxballs allows it on flexTPR. Skip oil-based lube, which Oxballs and Pipedream both rule out and which weakens latex condoms.

Do penis sleeves feel good for your partner?

That's what they're sold on, the extra length and girth on the outside. It depends on the partner and the size, so start with a modest one, an inch or two, use plenty of water-based lube on the outside and ask as you go.

How do you clean a penis sleeve?

Right after use. Silicone takes warm water and mild soap, and Pipedream's Elite silicone can be boiled for three minutes, though not its vibrating extender. TPE and TPR take what the maker says, usually warm water and a mild toy cleaner. Let it dry completely and store it apart from other toys. The cleaning guide covers every material.

Can you wear a condom with a penis sleeve?

Yes, on the outside, and a fresh one for each partner if the sleeve is shared, as with any toy. Use water-based lube with latex. Perfect Fit says its SilaSkin sleeves aren't compatible with latex, so check the maker's care notes.
